Introduction. This is LP Account, your free and private place for benefits, health and ID records. Here's how to use it, step by step.

Create an account. To create an account, all you need is a mobile phone. Add your name, number and birthday, and we text you a code. No password needed.

Sign in. For this tour, we're signed in to a sample account for the Rivers family, so everything you see is example data.

Your Care Hub. This is your Care Hub. The green badge is a promise: your record is private, and only you decide who sees it. Renewal alerts sit right at the top. Medicaid is overdue and SNAP is due soon, so let's get ahead of them.

Renew benefits. Recertifications shows every benefit with its case number and due date, so nothing sneaks up on you. Let's start SNAP. Your last renewal fills in the form for you: household, income, address and expenses. Just update anything that changed. When it looks right, submit. Your partner organization is notified, and you'll see a green confirmation.

Records. Records is your file cabinet. Keep your ID, pay stubs, leases and benefit letters in one place, ready when an agency asks.

Consent & sharing. Consent and Sharing shows exactly who can see your information, what they can see, and when their access ends. To share with someone new, pick the organization, choose what they may see, set how long, and add a reason. Review it once more, then grant consent. You're always the one who says yes. Changed your mind? Tap revoke, then confirm. Their access ends right away.

Activity log. Who accessed your data lists every view, share and change, with who did it and when.

Accessibility. Under Accessibility, turn on large text, high contrast, simple mode or reduced motion, whatever makes the app easier for you.

Local Partners. Want help in person? Local Partners lists OutPosts and service organizations, with what they offer and how to reach them.

For partner staff

Step-by-step guides for the partner workspace. Sign in at app.zion-pbc.com with your staff account. The menu is on the left on a computer.

Help a member sign up at your desk

The member approves sharing on their own. You never see or set their password.

  1. Open Assist a Member in the menu.
  2. Enter the member’s name and mobile number.
  3. Choose what you’re asking to share, and write why. The member sees your reason.
  4. Select Start request. You’ll see Request ready.
  5. Select Hand to member to approve and pass them the device, or ask them to sign in on their own phone.
  6. The member enters their mobile number, types the code we text them, then chooses I agree to share or No thank you.
  7. Take the device back. Under Recent requests, each request shows Waiting, Approved, Declined, or Expired.

Work your consented caseload

You only ever see members who chose to share with your organization, and only what they allowed.

  1. Open Caseload in the menu.
  2. Search by name, or filter by Consented, Intake done, or Flagged.
  3. Select a member to see what they share with you and their active consent grants.
  4. Select Open full record for their case hub: benefits and renewal dates, care episodes, and documents they shared.
  5. Need something they haven’t shared? Use Request access, choose what you need, and explain why. The member decides.
  6. Use Central Management to see open cases and upcoming renewals across your whole caseload.

Every time you open a record, the member sees it in their activity log.

Add or remove staff (admins)

Only your organization’s admin can change who has access.

  1. Open Staff & Access under Admin. You’ll see everyone with access, their role, and when they last signed in. Inactive 30d+ marks anyone who hasn’t signed in for a month.
  2. To end someone’s access, select Deactivate next to their name, then Confirm deactivate. Their access to your members ends right away.
  3. Made a mistake? Select Reactivate, then Confirm reactivate. Their previous role comes back.
  4. Every 30 days, confirm your team with I’ve reviewed my team. After 60 days without a review, adding new staff is paused.
  5. To add a new staff member, email support at zion-pbc dot com with their name, work email, and role. We’ll set up their access.
